Correspondence (letter and email) between the HIQA CEO and HIQA Director of Healthcare Regulation – and the following parties – in regard to governance of Letterkenny University Hospital’s gynaecology service, and quality and safety of same: HSE CEO; HSE Chief Operations Officer; HSE Director of Acute Hospitals; CEO of Saolta University Health Care Group; Clinical Director of Saolta University Health Care Group; Director of the Women’s and Children’s Managed Clinical and Academic Network, Saolta University Health Care Group; General Manager of Letterkenny University Hospital; Minister for Health; Secretary General at the Department of Health; and Chief Medical Officers at Department of Health. 

The period of the correspondence would be 1 September 2021 to 5 April 2022.
