The Patient Safety Act requires all health services providers, as defined in the Patient Safety Act, other than those that carry on the business of a designated centre or those that provide a health service in an approved centre regulated by the Mental Health Commission to notify HIQA of notifiable incidents. This document provides guidance on the notifiable incidents to be reported to HIQA and the manner in which they are required to be submitted.
