Since January 2019, HIQA, as the competent authority for regulating medical exposure to ionising radiation in Ireland, receives incident notifications of significant events arising from accidental or unintended medical exposures.
As part of its role, HIQA is responsible for sharing lessons learned from significant events. This report presents an overview of the findings from these notifications from the service providers (undertakings) and aims to share learning about these incidents and the investigations carried out by undertakings.
